General Information
This document outlines the terms and conditions for using the services of The Wonder Vet Pet Clinic. By booking an appointment, you agree to these terms.
Appointments & Punctuality
- Please arrive 5–10 minutes early for your scheduled appointment to allow time for check-in.
- OPD (Out-Patient Department) consultations can vary in duration. We appreciate your patience as we provide the necessary care to each pet.
- This appointment form is intended for non-emergency OPD consultations only.
- Prior notice: Prior appointment is required for all cases except emergencies.
- Being late: If an owner is more than upto 30 minutes late for the scheduled time, there is a rule to reschedule the appointment.
- Multiple people are prohibited from entering: It is best not to allow more than one owner with the animal inside the laboratory or testing room, so that doctors and technicians can work in peace.
Terms & Conditions for Appointment
- Guardian Communication Policy: For the safety and consistent care of your pet, only the pet's legal guardian (as indicated on this form) is authorized to contact and communicate with The Wonder Vet regarding appointments, treatments, or any other related matter.
- Follow-Up Communication Schedule: Post-appointment follow-up calls with the veterinarian are permitted only during the following designated time windows:
- Monday to Friday: 8:00 AM – 10:00 AM, 4:00 PM – 6:00 PM, and 8:00 PM – 11:00 PM (Local Time).
- Follow-Up Limitation: To ensure fair access and effective time management, single-pet guardians are limited to one (1) to two (2) complimentary follow-up calls per week. Any additional calls within the same week will be considered an Online Consultation and will be charged as per the clinic’s standard consultation fees.
Cancellations and Emergencies
- Cancellations must be communicated to the clinic at least 24 hours prior to the scheduled appointment time.
- For any pet emergencies, please call us immediately and proceed to the nearest emergency veterinary clinic. Do not use this form for critical situations.
Payment Policy
- All fees for services are due at the time services are rendered.
- For appointments booked online, a non-refundable booking fee is required to confirm your slot. This fee will be adjusted against your final bill.
- We accept cash, credit/debit cards, and online payment methods.
Owner Consent and Responsibility
- By signing the appointment form, you confirm that you are the legal owner or authorized agent for the pet(s).
- You give your full consent to The Wonder Vet Pet Clinic to provide medical treatment, diagnostic tests, and/or surgical interventions as deemed necessary by our veterinary professionals.
- You acknowledge that you understand the associated risks and potential consequences of veterinary care, as per government regulations.
- You agree to be responsible for all payment terms and associated fees for the services provided.
Liability
The Wonder Vet Pet Clinic and its staff will not be held liable for any unforeseen complications that may arise during or after treatment, provided that all standard care procedures were followed.
